At Midwest our passion is personal. Many of our team members, from direct care staff to our leadership team, have first-hand experience with Autism & ABA therapy. Because we’ve been there, we’re dedicated to giving our families the experience and services they deserve.
Each child in our center will be equipped with a team of providers based on their needs and BCBAs recommendation. While some skills will be developed in a group setting, the majority of therapy will take
place in a 1:1 setting.
Your Child’s Team Will Include:
Submit our online enrollment form. Our staff will get in touch to verify your info and answer your questions.
Step 3: Submit Enrollment Materials
Send us your completed intake packet (we'll provide this) as well as copies of your insurance card, your child's full diagnostic evaluation, and a referral for ABA therapy from your child's pediatrician. Materials can be faxed or emailed to [email protected].
Step 4: Develop Your Treatment Plan
Once we have your enrollment information, a provider will contact you to schedule your child's intake assessment. At the assessment, you'll meet your care team, discuss goals and, take a tour of the center (if you want to take a tour sooner let our staff know!)
Step 5: Start Learning
After the intake assessment, we will submit your child's treatment plan to insurance and get authorization to start therapy!
